One of the duties of the township trustee is to provide public assistance (poor relief) to those individuals
who may find themselves in need of assistance due to circumstances beyond their control

1. You must be a resident of Worth Township
2. Must be in need of basic necessities
3. Must have little or no income or assets
4. Must be willing to help yourself as much as possible

How do you file for public assistance?
Step #1 - Complete the pre-application online form below.
Step #2 - You will be contacted by the Township Trustee upon verification of your information.
Step #3 - If the Trustee verifies your eligibility, you will be asked to complete the full application found at the bottom of this page,
and return it to the Township Trustee by one of the following methods:
a) Print and complete the attached PDF form and contact the trustee (phone or mail) for an appointment.
b) Print and complete the attached PDF, scan and email completed form to the trustee.
c) Contact the Trustee for an appointment and pick up the form in person, and return upon completion.
